Mary Dahl suffers from a rare form of hypopituitarism, which prevented her growth and ability to age. She is already in her late 30s and still appears to be age five at most.
In part motivated by the medical bills that resulted from trying to find a treatment, Mary's mother and father decided to take steps to make their daughter an actress. Mary was absorbed into the Hollywood machine, the same as many children before her. She started in commercials, as most do. But the fact that she did not physically age made her an ideal "child star". When she hit puberty, rather than be given hormone treatments that might actually help her, Mary was instead given a starring role in the sitcom Love That Baby, and thus gained the moniker Baby Doll.
The show was moderately successful for several years, but once it started flagging in popularity, the producers added a new character -- Cousin Spunky. Baby Doll was often the butt of the joke during the final season of the show, and she resented the rest of the cast for going along with it. (And she also resented the other child actors being able to age.) When the show was cancelled, Mary tried to branch out into more dramatic roles since she was tired of the silliness and light topics of the sitcom world. Unfortunately, she overreached when taking the role of Lady Macbeth, and was ridiculed when people couldn't take her child-like appearance seriously in conjunction with the drama.
Mary Dahl went into seclusion for about ten years following that, watching with jealousy as her "TV family" of co-stars from Love That Baby went on to have much more successful careers and lead much happier lives than hers. Eventually, she decided that kidnapping them all and forcing them to repeat their old roles was the only thing to do. She wanted to recapture the brief time in her life when she felt happiest, and held them all prisoner on the abandoned soundstage of her old show. The actions of Batman and the first Robin thwarted her plans.
Mary tried to escape by fleeing the soundstage and hiding in a carnival but Batman apprehended her in the mirror room of a funhouse. She'd seen a mirror that distorted her reflection to appear like an adult, and had a breakdown. All she'd ever wanted to do was live a normal life... and that was denied to her by forces outside of her control.
